The Practice Guidelines , Patient Registration Form, and HIPAA Policy can be read prior to the first session and printed at home if you like. Please note that both clients and their parent/guardian should read these forms.
If we need to speak with another provider or reference regarding the reason why you are beginning to engage in psychotherapy with us, we need your written permission to obtain medical records or to speak with another person. Download, complete, sign and return the Authorization to Release Information form by fax or bring with you to your next appointment.
